The commonest pattern of onychomycosis and due in the majority of cases to dermatophyte infection. The lateral edges of the hyponychium provide a route of entry for the fungus which then spreads proximally under the nail plate, particularly adjacent to one or both lateral nail folds. The process often results in distal onycholysis.
