Electron microscopy shows thinning of the glomerular basement membrane. Diffuse thinning of the glomerular basement membrane is seen in many patients with familial haematuria and is characteristic of thin basement membrane nephropathy. This has been linked to abnormalities of the COL4A3 and COL4A4 genes.
