The classic form of stiff person syndrome consists of muscle rigidity with triggered painful proximal/axial muscle spasms which may lead to progressive disability and spinal deformities (e.g. exaggerated lumbar lordosis), electrophysiological evidence of continuous agonist and antagonist muscle activation, absence of other neurological conditions that could explain the symptoms with or without seropositivity for GAD65 autoantibodies.
