The majority of cases are sporadic and associated with a small-fibre neuropathy involving sensory and sympathetic nerves. It presents as episodic bouts of intense burning pain, heat and erythema involving the extremities lasting from a few minutes to several hours. Exercise and increase in temperature are precipitating factors. It is occasionally associated with certain drugs and it may rarely be familial due to a mutation in a protein involved in ion channel function.
