Electron microscopy shows characteristic irregular thinning, basket weaving and lamellation of the glomerular basement membrane in established disease. Abnormalities in type 4 collagen are found in Alports disease. Hearing loss and other features of Alports syndrome may be present and the combination of clinical and pathology observations may lead to that diagnosis
