The presence in the urine of glomerular origin red blood cells (RBCs). In microscopic haematuria the urine appears normal to the naked eye, but examination with a microscope shows a high number of RBCs. In macroscopic haematuria the urine is brown or red to the naked eye. Glomerular haematuria can usually be distinguished from urological haematuria by (phase contrast) microscopic examination when the RBCs are dysmorphic and heterogeneous in appearance, while they are normal in shape and homogeneous in urological haematuria which can also be associated with renal colic, clots and varying density within the urine stream. RBC casts may also be seen in glomerular haematuria.
