Hypertensive ischaemic leg ulcers (Martorell ulcers) manifest as painful necrotic ulcers typically located on the lateral aspect of the calf, which in 50% of cases are bilateral. Histologically there is subcutaneous stenotic arteriolosclerosis. All patients are hypertensive and over half are diabetic. Differentiation from pyoderma gangrenosum and calcific arteriolopathy is important as both prognosis and management are different.
