Encephalitis following infection with rubella virus usually presents 1 week after the typical maculopapular rash associated with rubella infection, although it can present without a rash. Specific IgM antibodies can be detected 4 days after the onset of the rash and can last up to 8 weeks after primary infection. In countries with high immunization rates, rubella infection and reinfection can be detected by a fourfold rise in rubella specific IgG antibody concentrations.
