Paraneoplastic movement disorders results from a targeted attack on the central nervous system as a consequence of a potentially effective tumour immune response initiated by onco-neural antigens derived from a systemic cancer. They are typically subacute, precede cancer detection and may be hypokinetic (parkinsonism [akinesia, bradykinesia, rigidity or postural instability]) or hyperkinetic (chorea, tremor [cerebellar/basal ganglia associated], dystonia or myoclonus). Investigations may reveal MRI abnormalities in the basal ganglia and an inflammatory spinal fluid. The neural autoantibody profile in the serum or cerebrospinal fluid predicts the cancer type [paraneoplastic movement disorders neural autoantibody positive definition].
