A mielopatia paraneoplásica resulta de um ataque direcionado ao sistema nervoso central como consequência de uma resposta imune tumoral potencialmente eficaz iniciada por antígenos onco-neurais derivados de um câncer sistêmico. A mielopatia pode ocorrer isoladamente ou como um componente de uma doença neurológica multifocal. Os pacientes geralmente apresentam mielopatia progressiva subaguda antes da detecção do câncer. As anormalidades de ressonância magnética específicas do trato são características, mas nem sempre estão presentes. O líquido cefalorraquianigeralmente é inflamatório. O perfil de autoanticorpos no soro ou líquido cefalorraquidiano prediz o tipo de câncer (consulte Mielopatia paraneoplásica: definição positiva de autoanticorpo neural)._x000D_ _x000D_ Os autoanticorpos neurais associados incluem:_x000D_ _x000D_ Anfifisina; ANNA-1 (anti-Hu) (auto-anticorpo nuclear antineuronal tipo 1); ANNA-2 (anti-Ri) (autoanticorpo nuclear antineuronal tipo 2); AQP4-IgG (autoanticorpos aquaporina-4); CRMP5 (anti-CV2) (proteína 5 do mediador da resposta da colapsina); Ma-1; Ma-2; PCA-1 (anti-Yo) (anticorpos, autoanticorpo citoplasmático de células purkinje tipo 1); PCA-2 (anticorpos, autoanticorpo citoplasmático de células de purkinje tipo 2);

Paraneoplastic myelopathy results from a targeted attack on the central nervous system as a consequence of a potentially effective tumour immune response initiated by onco-neural antigens derived from a systemic cancer. The myelopathy may occur in isolation or as a component of a multifocal neurological disease. Patients typically present with a subacute progressive myelopathy prior to cancer detection. Tract-specific MRI abnormalities are characteristic but not always present. Spinal fluid is usually inflammatory. The autoantibody profile in the serum or cerebrospinal fluid predicts the cancer type (see Paraneoplastic myelopathy: neural autoantibody positive definition). Associated neural autoantibodies include: Amphiphysin; ANNA-1(anti-Hu) (antineuronal nuclear autoantibody type 1); ANNA-2(anti-Ri) (antineuronal nuclear autoantibody type 2); AQP4-IgG (aquaporin-4 autoantibodies); CRMP5(anti-CV2) (collapsin response mediator protein 5); Ma-1; Ma-2; PCA-1(anti-Yo) (antibodies, purkinje cell cytoplasmic autoantibody type 1); PCA-2 (antibodies, purkinje cell cytoplasmic autoantibody type 2);
