Pemphigus vegetans is an uncommon form of pemphigus vulgaris. Onset occurs in middle age. Autoantibodies directed against desmosome components, chiefly desmoglein 3, cause intraepidermal clefting. The disease presents with superficial blistering usually of the flexures evolving into vegetating masses which become which eventually become dry, hyperkeratotic and fissured (Neumann type) or pustules that soon progress to vegetating plaques (Hallopeau type). Oral lesions are always present.
