Autoimmune autonomic neuropathies are immune-mediated autonomic nervous system disorders which may be limited, multifocal or generalised (pan-dysautonomia). Onset may be subacute or insidious and autonomic testing usually reveals abnormalities in sudomotor, adrenergic and cardiovagal function. It may be autoimmune or paraneoplastic and accompanied by neural autoantibodies. The prototypic disorder is an autoimmune ganglionopathy associated with nictonic ganglionic-acetylcholine-receptor-autoantibodies [see Autoimmune autonomic neuropathy, ganglionic acetylcholine receptor antibody positive definition]. Screening for cancer is appropriate in those at risk. Autoimmune dysautonomia may also occur with Guillain-Barre syndrome [see Guillain-Barre syndrome definition]. Exclusion of other etiologies is appropriate if the diagnosis is uncertain.
