Úlcera por pressão com perda total da espessura da pele. A gordura subcutânea pode estar visível, mas osso, tendão ou músculo não estão expostos. Esfacelo pode estar presente, mas não obscurece a profundidade da perda tecidual. Pode haver descolamento e tunelamento em estruturas adjacentes. A profundidade varia de acordo com a localização anatômica: as úlceras de pressão de grau 3 podem ser rasas em áreas com pouca ou nenhuma gordura subcutânea (p.ex., ponte nasal, orelha, occipício e maléolo). Em contraste, as úlceras de pressão de grau 3 podem ser extremamente profundas em áreas de adiposidade significativa.
