Mutismo seletivo é caracterizado por seletividade consistente para falar, de modo que uma criança demonstra competência linguística adequada em situações sociais específicas, tipicamente em casa, mas, de forma consistente, fracassa para falar em outras, tipicamente na escola. O distúrbio dura pelo menos um mês, não é limitado ao primeiro mês de escola e é de gravidade suficiente para interferir com o desempenho educacional ou com a comunicação social. O fracasso para falar não se deve a uma falta de conhecimento ou desconforto em relação ao idioma falado que é necessário na situação social (p. ex.,o idioma falado na escola é diferente do idioma falado em casa).

Selective mutism is characterised by consistent selectivity in speaking, such that a child demonstrates adequate language competence in specific social situations, typically at home, but consistently fails to speak in others, typically at school. The disturbance lasts for at least one month, is not limited to the first month of school, and is of sufficient severity to interfere with educational achievement or with social communication. Failure to speak is not due to a lack of knowledge of, or comfort with, the spoken language required in the social situation (e.g. a different language spoken at school than at home).
