Outro transtorno parafílico envolvendo indivíduos que não consentiram é caracterizado por um padrão persistente e intenso de excitação sexual atípica - manifestada por pensamentos, fantasias, impulsos ou comportamentos sexuais - no qual o foco do padrão de excitação envolve outros que não querem ou não são capazes de consentir, mas que não está descrito especificamente em outra categoria dos transtornos parafílicos (p. ex., padrões de excitação envolvendo cadáveres ou animais). O indivíduo tem que ter colocado em prática esses pensamentos, fantasias ou impulsos ou os mesmos lhe gerarem sofrimento significativo.
