Transtorno distímico é caracterizado por um humor depressivo persistente (i.e., com duração de 2 anos ou mais), durante a maior parte do dia, na maioria dos dias. Em crianças e adolescentes, o humor deprimido pode se manifestar como irritabilidade pervasiva. O humor deprimido é acompanhado por sintomas adicionais, como diminuição marcante do interesse ou prazer nas atividades, redução da concentração e atenção ou indecisão, baixa autoestima ou culpa excessiva ou inadequada, desesperança quanto ao futuro, sono perturbado ou aumento do sono, diminuição ou aumento do apetite, ou baixa energia ou fadiga. Durante os primeiros 2 anos do transtorno, nunca houve um período de 2 semanas durante o qual o número e a duração dos sintomas fossem suficientes para preencher os critérios diagnósticos para um episódio depressivo.
