Transtorno delirante é caracterizado pelo desenvolvimento de um delírio ou conjunto de delírios relacionados, tipicamente persistindo por pelo menos 3 meses e frequentemente por muito mais tempo, na ausência de um episódio de humor depressivo, maníaco ou misto. O conteúdo dos delírios varia entre indivíduos, mas é tipicamente estável para cada indivíduo, embora os delírios possam evoluir com o tempo. Outros sintomas característicos de esquizofrenia (ou seja, alucinações claras e persistentes, sintomas negativos, pensamento desorganizado ou experiências de influência, passividade ou controle) não estão presentes, embora várias formas de distúrbios perceptivos (p. ex., alucinações, ilusões, identificação incorreta de pessoas) relacionados com o tema do delírio ainda sejam condizentes com o diagnóstico. Exceto por ações e atitudes diretamente relacionadas com o delírio ou sistema delirante, o afeto, a fala e o comportamento tipicamente não são afetados. Os sintomas não são uma manifestação de outra condição médica (p. ex., um tumor cerebral) e não se devem ao efeito de uma substância ou medicamento no sistema nervoso central (p. ex., corticosteroides), incluindo efeitos de abstinência (p. ex., abstinência de álcool).

Delusional disorder is characterised by the development of a delusion or set of related delusions, typically persisting for at least 3 months and often much longer, in the absence of a Depressive, Manic, or Mixed mood episode. The delusions are variable in content across individuals, but typically stable within individuals, although they may evolve over time. Other characteristic symptoms of Schizophrenia (i.e. clear and persistent hallucinations, negative symptoms, disorganised thinking, or experiences of influence, passivity, or control) are not present, although various forms of perceptual disturbances (e.g. hallucinations, illusions, misidentifications of persons) thematically related to the delusion are still consistent with the diagnosis. Apart from actions and attitudes directly related to the delusion or delusional system, affect, speech, and behavior are typically unaffected. The symptoms are not a manifestation of another medical condition (e.g., a brain tumour) and are not due to the effect of a substance or medication on the central nervous system (e.g. corticosteroids), including withdrawal effects (e.g. alcohol withdrawal).
