O sinal característico da hipocalcemia aguda é a tetania, que é caracterizada por irritabilidade neuromuscular. Os sintomas de tetania podem ser leves (dormência perioral, parestesias de mãos e pés, cãibras musculares) ou graves (espasmo carpopedal, laringoespasmo e convulsões focais ou generalizadas, que devem ser distinguidas das contrações musculares tônicas generalizadas que ocorrem na tetania grave). Outros pacientes apresentam sintomas menos específicos, como fadiga, hiperirritabilidade, ansiedade e depressão, e alguns pacientes, mesmo com hipocalcemia grave, não apresentam sintomas neuromusculares. Os fatores que determinam a variação na frequência e gravidade dos sintomas incluem situação do equilíbrio ácido-básico (hipocalcemia e alcalose agem sinergicamente para causar tetania), hipomagnesemia e o nível sérico de potássio.

Other patients have less specific symptoms such as fatigue, hyperirritability, anxiety, and depression, and some patients, even with severe hypocalcaemia, have no neuromuscular symptoms. Factors that determine the variation in frequency and severity of symptoms include acid-base status (hypocalcaemia and alkalosis act synergistically to cause tetany), hypomagnesaemia, and potassium balance.
