+A bouba terciária se desenvolve em <10% dos indivíduos infectados não tratados após um intervalo de 5 anos ou mais. As lesões cutâneas em estágio avançado são caracterizadas por nódulos gomatosos com destruição de tecido necrótico, seguido por cicatriz debilitante e contratura. A osteíte destrutiva pode resultar em ulceração do palato e da nasofaringe ("gangosa") ou arqueamento da tíbia (canelas de sabre). A periostite hipertrófica em localizações periarticulares pode levar à exostose da maxila paranasal ("goundou").

The late stage skin lesions are characterised by gummatous nodules with necrotic tissue destruction, followed by debilitating scarring and contracture. Destructive osteitis can result in ulceration of the palate and nasopharynx (‘gangosa’), or bowing of the tibia (sabre shins). Hypertrophic periostitis at periarticular sites can lead to exostosis of the paranasal maxillae (‘goundou’).
