O padrão mais comum de onicomicose e decorrente na maioria dos casos de infecção por dermatófitos. As bordas laterais do hiponíquio fornecem uma via de entrada para o fungo que então se espalha proximalmente sob a lâmina ungueal, particularmente adjacente a uma ou ambas as pregas ungueais laterais. O processo frequentemente resulta em onicólise distal.

The commonest pattern of onychomycosis and due in the majority of cases to dermatophyte infection. The lateral edges of the hyponychium provide a route of entry for the fungus which then spreads proximally under the nail plate, particularly adjacent to one or both lateral nail folds. The process often results in distal onycholysis.
