Hemiballism, characterized by unilateral sudden, violent, flinging, or ballistic high amplitude movements, in the setting of cerebral infarct or stroke. Stroke is the most common etiology of acute onset hemiballism, with the most common infarct territories being the MCA (middle cerebral artery), PCA (posterior cerebral artery), and small vessels associated with lacunar stroke.
