Chronic primary chest pain syndrome is chronic retrosternal pain that is associated with significant emotional distress or functional disability. The distinct anatomical location is compatible with typical referral pain patterns from oesophageal origin or other organs of the chest. The pain must be present for 3 months with a symptom onset at least 6 months before the diagnosis with a frequency of at least once a week. The symptoms are not better explained by reflux disease, other mucosal or motor processes, cardiac causes, heartburn or a diagnosis of chronic secondary visceral pain. Chronic primary chest pain syndrome is multifactorial: biological, psychological and social factors contribute to the pain syndrome. The diagnosis is independent of identified biological or psychological contributors unless another diagnosis would better account for the presenting symptoms.
