Eczema atópico infantil é definido como aquele presente durante o primeiro ano de vida. Manifesta-se tipicamente entre o segundo e sexto mês: cerca de 50% dos indivíduos com eczema atópico têm a primeira manifestação na infância. A face e áreas não flexurais são mais comumente afetadas. A área da fralda tende ser relativamente poupada. Acometimento das dobras dos membro, como é típico no eczema tópico da infância, também é frequentemente observado em bebês.
