Todos os critérios diagnósticos para transtorno de oposição desafiante sem irritabilidade-raiva crônicas são preenchidos. Além disso, o indivíduo apresenta características que são, às vezes, denominadas “insensíveis e sem emoção”. Essas características incluem falta de empatia ou de sensibilidade aos sentimentos dos outros e falta de preocupação com o sofrimento dos outros; falta de remorso, vergonha ou culpa em relação ao próprio comportamento (a não ser quando motivado por ter sido preso), indiferença relativa à probabilidade de punição; falta de preocupação em relação a desempenho ruim na escola ou no trabalho; e expressão limitada das emoções, particularmente emoções positivas ou amorosas em relação aos outros, ou expressão apenas de maneira que parece superficial, não sincera ou instrumental. In addition, the individual exhibits characteristics that are sometimes referred to as ‘callous and unemotional’. These characteristics include a lack of empathy or sensitivity to the feelings of others and a lack of concern for others’ distress; a lack of remorse, shame or guilt over their own behaviour (unless prompted by being apprehended), a relative indifference to the probability of punishment; a lack of concern over poor performance in school or work; and limited expression of emotions, particularly positive or loving feelings toward others, or only doing so in ways that seem shallow, insincere, or instrumental. This pattern is pervasive across situations and relationships (i.e., the qualifier should not be applied based on a single characteristic, a single relationship, or a single instance of behaviour) and is persistent over time (e.g., at least 1 year).

These characteristics include a lack of empathy or sensitivity to the feelings of others and a lack of concern for others’ distress; a lack of remorse, shame or guilt over their own behaviour (unless prompted by being apprehended), a relative indifference to the probability of punishment; a lack of concern over poor performance in school or work; and limited expression of emotions, particularly positive or loving feelings toward others, or only doing so in ways that seem shallow, insincere, or instrumental.
